Athletic Clearance Form

For the 2023-2024 School Year, we will be going with an online process to submit your physicals. This will be an easier way to clear your athletes to participate in sports. Please upload the necessary paperwork to www.AthleticClearance.com 

All athletes must be cleared prior to tryouts and practice.

Returning athletes use the same account and use the the "Choose Existing" dropdown. All contact information will auto-populate and they have a "Choose Existing" file library on the files step to allow for easier uploads.
